Job Requirements - Active LPN license in the state of Alabama (state licensure required)
- Minimum 1 year of experience in Long-Term Care, Skilled Nursing Facility, or similar setting
- Ability to work 12-hour day shifts and meet schedule requirements
- Strong communication, teamwork, and documentation skills
- Commitment to compassionate resident-centered care and adherence to care plans
- BLS (Basic Life Support) preferred or as required by facility
Responsibilities - Provide direct nursing care to residents in accordance with individualized care plans
- Administer medications, treatments, and pain management as prescribed (medication administration)
- Monitor, assess, and document resident health, vital signs, and progress
- Communicate changes in condition to the interdisciplinary care team and physician
- Support residents with activities of daily living (ADLs) and promote functional independence
- Participate in care planning, transfer/discharge processes, and quality improvement initiatives
