Job Requirements / Qualifications - Active and valid LPN license in the state of Delaware
- Experience in long-term care, skilled nursing, or similar LTC setting is preferred
- Comfortable working 8-hour evening shifts and completing a full 6-week travel assignment
- Strong communication, teamwork, and resident-centered care skills
- Experience with medication administration, charting, and electronic health records (EHR) is a plus
- Familiarity with facility policies, infection control, and state LTC regulations
Responsibilities - Provide compassionate, high-quality nursing care to residents in a long-term care/skilled nursing facility
- Administer medications and treatments per provider orders and facility protocols (medication administration)
- Monitor, assess, and document resident status, vital signs, wounds, and care progress in the EHR
- Collaborate with RNs, CNAs, therapists, and interdisciplinary team members to coordinate resident care
- Support admissions, transfers, and discharge processes as needed during the evening shift
- Maintain compliance with facility policies, infection control procedures, and Delaware state regulations
